What is 123 Dental Care?
 

123 Dental Care is a dental referral organization that offers memberships in its discount dental plan. Members can save 25% to 65% on all restorative and cosmetic work (fillings, crowns, braces, etc.) and up to 80% on preventative work (teeth cleaning, x-rays, etc.) performed by a general dentist. Specialist’s fees are discounted 25%.

Is 123 Dental insurance?

123 is not insurance or a health care organization. Unlike expensive insurance, 123 offers immediate access to our dental providers. All pre-existing conditions are covered except orthodontic treatment in progress. There are also no limits on visits or services and 123 includes Vision, Prescription Drug and Chiropractic plan benefits at no extra charge. There are also no claims to file by the member or dental office.

How do I pay my Membership Fee?


123’s low membership fee is directly tied to bank and accounting efficiency. These low monthly or quarterly payments are made ONLY by electronic bank draft (checking or savings account) or by debit or credit card. If you do not have the ability to secure a bank account, your membership may be made by paying annually (one year in advance). The individual membership rate is $11.95 a month, and family membership is $19.95 a month. I recently re-married. Can I include my new spouse and his/her children on my Household Plan?


Yes. A “Household” membership is defined as anyone living within your physical residence including children, parents, and relatives, significant others, and all permanent residents of the household.

How do I use the Plan?

Within 10 to 14 working days of receipt of your application in our home office, you will receive:

- Member Information Guide

- Membership Identification Cards

- Provider Directory

Simply select a provider and call to set an appointment. Remember to identify yourself as an AmeriPlan TM patient with your assigned ID number (printed on your ID card). On the day of your visit present your ID Card to any AmeriPlan TM provider and pay the reduced fee . . that’s it! No claim forms to file, no headaches or hassles!

This sounds too good to be true. Explain to me why a dentist would be willing to reduce his fee?

How soon can I use my plan?

Do I need to list my Social Security Number to enroll as a Member?

No. You may use any nine-digit number that you would like (and can remember) if ask to verify your account by a provider.

Do I have to use an 123 dentist?

Yes, just as you would with other plans -- with one significant difference. The vast majority of our providers have teamed up with 123 due to being referred from their own patients or other providers. What this means is that people love and respect our doctors, and we’re confident that you will too.

I called my Pharmacy to see if they accept 123, and they weren’t sure. What can I do?

Once you receive your Membership ID Card, take it physically to your Pharmacy and point out the Prescription Drug logo that appears on the card. Pharmacies across the nation recognize this as one of the largest pharmacy network companies in the country – and you’ll find that most honor the plan.

A special 1-800 number is included in your membership kit to refer your favorite pharmacy, dentist, vision center and chiropractor.

Can I use this with my existing Dental Insurance Plan?

Yes, you can work as a supplement to a fully insured plan. The fully insured plan is always considered the primary coverage that the dentist will use in calculating the member’s 123 benefits. What this means is:

- If the procedure being done is not covered at 100% of the member’s fully insured dental plan, the remaining balance will not be greater than the negotiated 123 fee.

- 123 may be used as the primary benefit only if the procedure performed is not covered under the fully insured dental plan.

123 does not work with a managed care plan (HMO/PPO), as they are discounted fee-for-service plans also.
